Prior to participation, a Fitness Center Policy/Release form must be completed and on file.
For their own safety, no children under the age of 12 will be admitted. In, addition, children ages 12-17 must be supervised by a parent or guardian who is present.
Do not use equipment until you thoroughly read and understand the instructions.
Injuries and defective equipment are to be reported to the front desk immediately.
Cords, plugs, switches and settings (including thermostat) on all fitness center equipment (including TV’s) are to be handled by staff only. ANY VIOLATION WILL RESULT IN POSSIBLE TERMINATION OF MEMBERSHIP.
Proper workout attire must be worn. Clothing must be worn in the saunas:
a. No spandex or tight fitting clothing, unless worn under loose fitting T-shirts, and/or sweats.
b. No low cut, or otherwise revealing clothing (including spaghetti straps). Midriff must be covered.
c. No street shoes
d. Shirts must be worn.
Participants must provide their own towels for the shower. DO NOT USE PAPER TOWELS TO DRY OFF after SHOWER.
No food or drinks allowed- except water in plastic containers with lid.
Please wipe down all machines after each use, disinfectant and paper towels are available for your use.
Use of machines is limited to 30 minutes while others are waiting. Weights are to be shared in between sets.
Equipment must be returned to its original position after use. Re-rack all dumbbells and take off heavy weights when finished.
Do not leave personal belongings unsupervised. Gilead Healing Center is not responsible for lost or stolen items.
Any misconduct or violation of the rules can result in loss of Fitness Center privileges.
FOB sharing will not be tolerated and will result in immediate termination of membership. All fees are nonrefundable. Neither I nor the person with whom I shared the FOB will be allowed to obtain a membership in the future.
Key Fob – initial FOB will be provided with membership. Replacement key FOB will results in a $5.00 charge due at the time of replacement. Upon cancellation of membership, key FOB must be returned. If the FOB is not returned a $5.00 charge will apply.
Waiver/Release of Liability
I intend to use some or all of the health and fitness activities, facilities, programs and services offered at or by Gilead Healing Center and agree to abide by all applicable rules and regulations.
By, and in consideration of, my participation in any of the health/fitness activities of Gilead Healing Center, or use of the health and fitness center or facilities of Gilead healing center, with the permission of Gilead Healing Center:
I understand that exercise may expose me to above normal risk or injury. I agree that I am responsible for my own safety.
I agree that Gilead Healing Center, it’s associates, staff, employees, trainers, agents, and volunteers (hereafter all referred to as “Gilead Healing Center”) shall not be liable for any damages or loses arising from any personal injuries sustained by me as a result of my participation in any program or use of the facilities of Gilead Healing Center.
By execution of this waiver/release of liability, I fully and forever discharge and release Gilead Healing Center from any and all claims, demands, damages, rights of action, or causes of action (whether based on a claim of negligence or otherwise, present or future known or unknown, anticipated or unanticipated, arising out of, or in any way related to, my participation in these activities, or use of these facilities, or any related physical training, exercise, activities, instruction, counseling or guidance, and agree not to sue Gilead Healing center for any injuries, damages, or losses (including loss or theft of personal property) incurred or sustained by me.
If any term or provision of the waiver/release of liability, or the application to any person or circumstance, shall, to any extent, be invalid or unenforceable, the remainder of this waiver/release of liability, or the application of the term or provision to persons or circumstances other than those as to which it is held invalid or unenforceable, shall not be affected, and each term and provision shall be valid and be enforced to the fullest extent permitted by law.
I agree to submit any and all controversies arising out of, or in any way related to, this waiver/release of liability (and the subject manner of this waiver/release of liability) to binding arbitration with the Christian conciliation service of central Michigan. The arbitration shall be conducted pursuant to the rules of the Christian conciliation service of central Michigan. If the Christian conciliation service of central Michigan is no longer available to arbitrate a dispute, then the Christian Legal Society shall recommend an Alternate Dispute Resolution Arbitrator, whose rules shall apply. Any decision will be final, binding on all parties and a judgment of any Circuit Court may be rendered upon the award.
I have read this agreement before signing it. I acknowledge the above terms, understand and agree to abide by them.
